It is a special permit to handle food for public consumption. It is required to cook serve or in any way handle food for public sale. All states have it.

The card is a permit required of anyone working with food, and it certifies that the permit holder has gone through training regarding food safety and sanitary practices. Sometimes it also includes a section on what to do if someone is choking. Other states such as Washington and Indiana also require the card.
