Each state in the US has very different policies for birth certificates, not to mention very different styles for the birth certificates. In some cases you may not be able to change your name, others may require going to court.

You would need to contact the department in the state where the birth certificate was issued. Usually, if it is within a certain time frame from birth you can make the change without any problems. Otherwise, it might require a legal / court name change.
