She had formula for the first time today. After three hours, it all came back up.

If the vomiting is extremely projectile, it could be Pyloric Stenosis. It's most common in the first born male, but no baby is unable to have it... most likely a formula thing.

It could be an allergic reaction to the type of formula (has it expired?), the bottle could be dirty, or, she could just be sick.

It could be that she is having some kind of allergy to something in the formula. Also, it may have caused her to have some reflux.
