Eevee evolves into one of several forms depending on the type of "stone" item you use on it. Vaporeon can be evolved from the pokemon Eevee by using a water stone on it.

You can't, not in the wild. You'll have to evolve an Eevee to get one in Platinum. If you go to Hearthome City, one of the NPCs there will give you the Eevee. Alternatively, if you go to Rt. 214, there is a trainer there that uses Vaporeon, but you can't catch it.
