During a Carotid endarterectomy a surgeon will remove blockage from the carotid arteries in the neck. A Carotid Endarterectomy usually takes 1-2 hrs and is done under general anesthesia.

This would probably involve making an incision in your neck and into the carotid artery to remove plaque from the artery. The artery would first be clamped to prevent blood loss and then the surgeon would be able to scrape out the built up plaque.

If you are talking about having the plaque removed then you would be placed under general anesthesia and the neck will be opened up, the the artery would be opened up and the plaque would be scooped out.

If this is not stent surgery, your doctor will make a small incision to the carotid to remove plaque buildup. They may temporarily reroute the blood flow. This is a common surgery, and older than stenting. While no surgery is routine, this is done with regularity and is fairly straightforward.
