Mildew is a type of fungus that grows in moist and warm places and can grow on most types of fabric, wood, paper, leather, linoleum and paint. As the mildew grows, it leaves a sour musty smell. Some common causes are poor air circulation, low lighting and warm humid temperatures.

Mildew is usually paired with bacteria. If the cleaner you used removed the mildew and stain, but left the odor, chances are there is still some bacteria left. Try using bleach, Mr.Clean with Febreeze, or Lysol. I have had success removing not just the mildew, but all of the odor and bacteria with all of these items.

Mildew odors are caused by the existence of mold on fabrics. They can became a problem in areas that are damp, warm, dark or where air is not circulating well.

One of the reason that causes mildew odors would include poor ventilated areas wherein moisture is trapped instead of evaporating. Another is excess moisture in an enclosed area.
