Pinata's are always a fun thing for kids, thought the downside to this is that it can get dangerous with a blindfolded child swinging a bat around. Things such as musical chairs are fun, if you have disabled children at the party then I suggest bingo or something they can do from their seats.

When I was his age I loved Musical Chairs (Where you take one chair away and everyone has to sit down when the music stops until there's only one left)... and Charades. Everyone loves Charades - where you mime a book, movie, TV show... and everyone else has to guess.

For my 4 year old nephews birthday I created cardboard cars and put each kid in them. They then competed in an obstacle race.

You could have a treasure hunt and hide different items outdoors and whoever finds the most items wins a prize. Also you can have a pinata that matches your theme.
