Mandated benefits are done by federal law, state law, or both. These laws are designed as guidelines for insurance companies. They are required to cover specific health conditions. What's mandated varies from state to state, there are over 2000 state laws. Some examples are substance abuse treatment, contraception, maternity services, smoking cessation, prescription drugs and in-vitro fertilization.

Mandated benefits are those that are required to be in group health care plans. Some of the situations that can be included are autism coverage, in vitro fertilization, HPV vaccine screening, coverage for domestic partners, coverage for grandchildren, coverage for dependent students until they are 30 years old, and more.
