Go to the post office and pick up an application with directions/requirements. Assemble all documentation, complete the application, then make an appointment at the post office to submit it.

There is an application that has to be filled out. You must provide evidence that you are a citizen of the United States, you must provide identification, there is a fee that is required to be paid and you must have two photos. These are just a few things that are required to get an United States passport.

Fill out form DS-11 (available at a post office). Submit proof of citizenship (copy of birth certificate, etc), 2 passport grade photos, and pay $135.
