The United Council on Welfare Fraud has an online site that lists the proper authorities in each state. You can also call the Department of Social Services and ask for the appropriate contact. Each state has a dedicated office that handles matters of fraud with regard to various state and federal assistance programs.

You can report it directly to the agency that provides the funds. Nearly every governmental agency that supplies some fort of assistance has an investigative component of the agency. Simply notify the state or federal agency that provides the service (HUD, VA, etc. ), and ask to speak to the investigative division. They'll do the work from there.
