If you are female, generally speaking a plastic surgeon COULD operate but probably will not see the point. If you are male, your doctor should test you for gynecomastia and choose where to proceed from there. Unless you have gynecomastia, puffy nipples usually aren't a problem that needs fixing.

Puffy nipples is a common form of gynecomastia that is most often found in young adults and can be treated with surgery. This procedure takes around 2 hours and recovery time is short, usually being less than one week. This procedure is considered highly effective in treatment of this condition.

That depends on what is ment by "puffy nipples", anytime you have a concern about anything/ any system in/on your body you should seek medical attention. Many women who have recently had a child may have enlarged nipples which helps with breast feeding, if you are concerned make an appointment with your primary healthcare provider.

Unless there is a specific affliction or a strange gathering of fluid, then the answer is no unless you want to consider some original plastic surgery which is quite possible.
