I had to leave my job due to an injury. What is the best answer for why I left?

You can put down the reason you left as being personal if it is an injury that you would rather not discuss with anyone. I would also use that if the injury was an on the job injury. Otherwise, you can explain that you left the job for health related reasons.

Inform them that you left because an injury left you unable to perform the job duties. Then state that you are fully capable of meeting the current job requirements.
