How do you know when your clarinet should be refurbished?

Your clarinet should be refurbished when the pads start to erode or fall off, or when the cork between the sections begins to come off and the horn has difficulty assembling.

This is largely a matter of opinion, if your instrument is performing poorly it might be time to either refurbish it, at the least you can consult a music expert.

The short answer is when the keys stick or leak. If you take it anywhere near "an expert", he will say it needs new pads, springs, and anything else you can spend money on - especially if he has his own shop. Best thing is to ask an experienced player to try it, see if they think it needs attention.
