How do I get an Illinois handicapped parking permit?

I am moving to Illinois soon.

A physician must complete a form (available online) stating why you need a handicapped parking permit. The application is for a temporary tag or a permanent one. You then take the completed form to the BMV and get the tag.

As long as you have permission from your doctor detailing your reason to have one you can take it to the DMV and they will verify your information is legitimate and issue you one.

You need the application and physician certification form from the Illinois Secretary of State Office. Fill out your part, and have your physician fill out theirs. Mail to the Sec'y of State address listed on the form- there is no fee for the placard, there is a fee for special DMV plates. They will be mailed, display according to instructions.

The drivers license bureau will have paperwork that will need to be filled out by your physician. In most states it may be required to have an in state doctor fill it out. It could be handy to bring with your old state permit to further help your case. The license branch will then give permit.
