How do I get a copy of a Maryland birth certificate?

I need it for a passport.

There are several ways to get a birth certificate in MD. You can go to your local health department with your proper paper work and they can get you one right there. If you have time you can go on the website and print a form, fill it out and send away for it.There is a fee.

The most economical way to get a copy of a birth certificate is through the County of Frederick, 350 Montevue Lane, Frederick, MD 21702-8245 or phone (301) 600-1739. There is a fee. There are other online services, which charge high fees and take advantage of people who don't know they can order the record directly themselves.

For same day service, you may obtain a copy from the Vital Statistics Office in Baltimore, Maryland. There is also an application you may fill out to request a copy by mail. For either means, you must ensure that you have the proper identification.
