I think how you go about it depends on what is it that you are apologizing for. It it's serious it could be seen as an admission of guilt.

The best way would probably be to write a personal letter to the patient and mail it to his/her home. Phone calls may be a bit invasive for some people.

Sending a card is okay if you're not going to see them. However, the best thing you can do is to tell the patient that you are sorry to their face. Don't try to say something that seems rehearsed. Say what you are feeling and let the words come from your heart.

Sit the patient down and honestly explain what happened and assume responsibility without assessing blame. More likely than not the patient will appreciate your honesty.
