The first step is to contact the old employer. If that is not possible, then the next step is to file form 4506-T which will alllow you to get a transcript of the information on the old W-2 from the IRS free of charge.

Contact your old employer and request a copy of the W2 for that year. Companies are required to keep payroll records ( especially W2's ) for years.
