Green cards are not automatically granted; the process includes lengthy interviews. If the immigration officer doubts the sincerity of the marriage, the couple faces fraud charges.

Yes, people who are desperate do it. If they really need the money, and the other person really needs the green card, sometimes they reach a deal. It's not a good idea though.
