Ladies do not have to remove their hats during the national anthem. Mens hats are easy to remove but women may have pins, flowers etc making it difficult to remove.

Removing a hat during the national anthem is a sign of respect for America. If you are wearing a baseball cap, it is appropriate to remove your hat, whether you are male or female.

In a word, yes. In days gone by, a woman's hat was secured to her hair with pins and it's removal was impractical if not impossible. These days, almost everyone wears unisex hats such as baseball caps and they should be removed during the anthem, whether worn by a man, woman or child.

Speaking from a standpoint of pure etiquette, a lady is not required to remove her hat in any setting if she is wearing a formal hat, even when hearing the National Anthem or meeting the Queen of England. However, if it's not a formal hat (like a ball cap if you're out at a game) then it must be removed.
