Cavities that have not yet reached the nerve of the tooth are called carries, and these will not cause pain. When it does reach the root, then the pain is extremely severe. A dental infection that reaches the root is undeniable and known merely by the pain, if not by the likely inflammation.

If you are talking about having a simple cavity then yes it is possible to sometimes miss it as there may not be any pain when the cavity develops on the tooth itself. However once the cavity starts to develop closer to the root area or the gum are itself, you will feel it;any infection otherwise will be felt.
